Things to Remember, as You Begin the Process of Planning

What to Bring When You Meet the Funeral Director

Check List
Social Security Number (SSN, eg 123-45-6789)

Veteran's discharge papers or other records

Life insurance policies

At least one recent photograph (for cosmetic purposes)

Eyeglasses and/or jewelry (these items can be left with your loved one or removed and returned)

Clothing, including undergarments

List of 6-8 8 pallbearers, if applicable

Valued photographs for the Everlasting Memorial® video and multimedia scrapbook of family memories

   


What Your Funeral Director Must Know to Complete the Arrangements

Check List
Mother's full name (including maiden name)

Father's full name

Birthplace

Education, including diplomas, degrees, etc.

Work history

Church affiliation

Organization or memberships (civic or fraternal)

Special achievements

Names of surviving relatives (and cities where they live), including the following:

    • Spouse(s)
    • Parents
    • Children
    • Siblings
    • Grandchildren (and great-grandparents)
    • Grandparents
    • Other immediate family and/or special friends

What You Can Expect Your Funeral Director to Do for You

Check List
Arranging a suitable time/date for the service

Coordinating the service with the cemetery, crematory or disposition service (eg. spreading ashes at sea, etc.)

□ Arrangement and coordination with minister, priest or rabbi (eg. Ministry For Grief and Loss)
□ Arrangement and coordination with musicians, singers or other ancillary services (eg. doves, balloons, etc.)

Airline coordination and/or family transportation

Monument and inscription service, if necessary

Flowers

Everlasting Memorial® or other video archive and scrapbook

Newspaper notices/obituary

Filing the death certificate and ordering certified copies in your behalf

Arranging and/or filing for:

    • VA benefits
    • Social Security benefits
    • Life insurance benefits
